What is ACO REACH?

The Accountable Care Organization Realizing Equity, Access, and Community Health (ACO REACH) Model is an alternative payment model from the Centers for Medicare and Medicaid (CMS) to encourage primary care physicians (PCPs) and other eligible professionals to shift away from fee-for-service (FFS) compensation to value- based payments in primary care.

How is the ACO REACH Model different from other Value-Based Care (VBC) programs?

Improvements on Past Medicare Value-Based Programs

Enhanced revenue opportunities for PCPs, including stable monthly payments and shared savings

Reduced patient cost sharing and out-of-pocket expense for patients

Easier access to post-acute providers including skilled nursing and home health providers
