Dr. Mildred “Millie” Frantz is a family medicine doctor in Eatontown, New Jersey, and is affiliated with Monmouth Medical Center-Long Branch Campus. She received her medical degree from Rutgers Robert Wood Johnson Medical School and has been in practice for more than 20 years.
In 2005, she founded HealthCare for Life, LLC, to serve the community that she loves. As a long term resident of Monmouth County, she graduated with an MD from UMDNJ-Robert Wood Johnson University, completed an Internship at Mayo Clinic, and was chief resident at Penn State Hershey Medical Center.

Dr. Frantz feels privileged not only for the trust she receives from her patients, but also with having that opportunity to work alongside her very own family. She, her family, and the collective Healthcare for Life team are passionate about genuinely being there for their patients and staying involved in their care and wellbeing throughout their lives. They believe in preventative care, positive patient experiences, and personalized evidence-based medicine — viewing their patients as extended members of the HealthCare for Life family.

As a result of her experiences in Comprehensive Primary Care (CPC) models, her practice now regularly offers weekend and after hours services, including managing care needs that cannot be met during regular office hours, ensuring patients have sufficient medication through weekends, and offering consultations prior to any potential ED visits on her personal cell-phone number. Dr. Frantz and her team seek to prevent avoidable readmissions and make follow-up contacts with patients within seven days of hospital discharge.

In addition to managing her practice, Dr. Frantz has served as a medical consultant for the Township of Ocean-Department of Human Services since 2020. She also has served on NAVVIS’ Physician Governance Committee since 2021, where she provides oversight and strategic direction in an advisory role to assist in Population Management Policies and measures in conjunction with Horizon Insurance — New Jersey’s largest and oldest health insurer.

Prior to that, Dr. Frantz served on Horizon’s Patient Centered Medical Home Value Based Advisory Committee from 2015-2017. She provided feedback and guidance to shape the future of Horizon’s patient-centered programs, focusing on the evolution of value-based payment models and optimizing support of practice level transformation. Dr. Frantz continues to work with Tower Lodge SNF, where she served as Medical Director from 2013-2014.
