Pearl Health, a leading provider of primary care physician (PCP) enablement and risk management services, announced today that it has partnered with ViCare Health, an independent physician association, to help ViCare Health physicians transition to and succeed in value-based care for Medicare patients.
Pearl Health’s technology platform combines data science with an intuitive user experience to help PCPs deliver more proactive, whole-patient care by identifying and engaging with the individuals who are most likely to benefit from proactive care interventions.
